In 2026, after many years of hesitation and debate, France could take a major step forward by authorizing the medical use of cannabis. The result of an experiment that began several years ago, this breakthrough marks a crucial milestone for many patients seeking innovative therapeutic solutions. Let's take a look back at the major milestones in this process and the future implications of this legalization.

A long road to recognition of medical cannabis

The trial of therapeutic cannabis in France began in March 2021, under the supervision of the French National Agency for the Safety of Medicines and Health Products (ANSM). Initially scheduled to last two years, this pilot phase has been extended until the end of 2024 to assess the efficacy and feasibility of this treatment on a larger scale. In total, 3,000 patients suffering from severe conditions (epilepsy, neuropathic pain, adverse effects of chemotherapy, etc.) have benefited from innovative treatment to relieve their symptoms and improve their quality of life.

In recent months, however, the future of this experiment has raised serious concerns. To everyone's surprise, the government considered halting the program, threatening to deprive many patients of a treatment deemed promising. Faced with outrage from healthcare professionals and patient associations, the government ultimately opted to extend the experiment and prepared for its expansion, scheduled for 2026.

A strict and rigorous medical framework

Following on from this experiment, the legalization of medical cannabis in France will take place under very strict conditions. Only authorized physicians will be able to prescribe it, and only for specific pathologies when conventional treatments have not proven sufficiently effective. The forms administered (oils, dried flowers, capsules, etc.) will also be subject to strict control protocols to guarantee the product's safety for the patient.

Production and distribution will be handled by approved structures, which must meet stringent quality and traceability standards. With this in mind, the government notified the European Commission in March 2025 of its draft regulations to ensure full compliance with EU law. According to this draft, only cannabis-based medicines with specific authorization may be prescribed under this system.

Generalization planned for 2026: challenges and perspectives

While waiting for the widespread adoption of medical cannabis, the Ministry of Health and Access to Healthcare has extended the coverage of patients already enrolled in the experimental program until March 31, 2026. This transition aims to avoid any interruption in treatment and to continue clinical observations on the effectiveness of medical cannabis in various indications.

The legalization of cannabis for medical use in France illustrates a notable shift in the perception of so-called "alternative" treatments. Limited for the moment to strictly therapeutic use, this measure nevertheless reflects an awareness of the potential benefits of cannabis, similar to what is already happening in several European countries. Although challenges remain, particularly in terms of training healthcare professionals and raising public awareness, this progress could open up new perspectives for patients facing serious conditions or those resistant to conventional treatments.
